All incoming students are required to complete a health form prior to arriving to campus. The health form consists of three parts: health history, physical examination by a physician and immunization record. Click here to access the health form (it is listed as SOM Term 1 Health Form). You can start working with your physician and gathering all your information. You will receive a notification of where to upload your health forms prior to coming to campus.

The Health Form is comprised of two sections: Mandatory and Recommended.
Mandatory: This section includes the Personal History, Physical exam, TB screening, Mandatory blood
work, mandatory vaccinations and completion of the Meningococcal Meningitis Screening.
Students should submit supporting documents, as necessary. When submitting proof of specific prior vaccinations, Lot #s, MD signatures are not necessary if a vaccination record is included.
Recommended: Completion of this section is not required, however the student and personal physician
will determine if these additional recommendations should be completed and submitted.
